Please read and affirm each statement. This waiver applies specifically to the 2026 Christ Is King Flag Football program at Robinson Township Park.

I certify that I am the parent or legal guardian of each player named above and have authority to sign this agreement for them.

I understand that flag football and related warm-ups, drills, games, and use of the playing area involve inherent risks, including falls, collisions, contact with other participants or equipment, weather exposure, and other injuries that can occur during athletic activity. I voluntarily permit the player(s) named above to participate and accept those inherent risks.

To the fullest extent permitted by Michigan law, I release Christ Is King, its organizers, coaches, assistant coaches, and volunteers from liability for economic or noneconomic damages for injury or death resulting solely from the inherent risks of the 2026 Christ Is King Flag Football program. This release does not waive claims that Michigan law does not permit to be waived, including claims arising from negligence that causes or contributes to injury or death.

If I cannot be reached in an emergency, I authorize Christ Is King representatives to obtain reasonable emergency medical care for the player(s) named above. I understand that I remain responsible for related medical expenses.

I agree to conduct this transaction electronically. I understand that typing my full legal name and submitting this form constitutes my electronic signature and has the same effect as signing a paper document.
